Responsibilities:
• Assist clients with daily living activities, including bathing, grooming, and dressing
• Perform light housekeeping tasks and meal preparation
• Provide companionship and emotional support
• Follow individualized care plans to ensure client 

 • Document care and maintain accurate records

• Provide necessary forms and package all forms in preparation for service determination.
• Assist the individual in collecting, completing, gathering required information and documents for the services.
