COMMUNITY HEALTH WORKER RESOURCE PAGE

Community Health Worker use client-centered concepts and skills to work with a client to increase physical activity and daily living skills.  Our CHW's create a supportive and collaborative relationship with the client, ultimately empowering them to make sustainable changes in their physical activity habits.  The client needs, preferences and autonomy remain at the center of the intervention, leading to more successful and lasting results.  CHW play a vital role in using education to promote healthy behavior change within communities.
